Job summary

Christchurch Medical Practice in Dorset seeks an Advanced Nurse Practitioner to deliver autonomous, high-quality clinical care including face-to-face consultations, triage, and home visits when needed. Qualifications

• Registered General Nurse (RGN) with current NMC registration.
• MSc, PG(Dip) or MA in Advanced Clinical Practice preferred.
• Minimum 2 years post-registration experience in primary, community, or acute healthcare.
• Experience managing acute illness, long-term conditions, and minor ailments independently.
• Skilled in triaging, diagnosing, prescribing, and referring both adult and paediatric patients.
• Competent in interpreting diagnostic tests and routine clinical procedures.
• Proficient with clinical systems such as SystmOne.
• Full UK driving licence with access to a vehicle or alternative transportation.
• Working towards or completed Advanced Clinical Practice at Masters level.
• Experience in inter-agency working across health networks.
• Experience in clinical governance, risk assessment, and health promotion.

Job summary

We are seeking a highly skilled and motivated Advanced Nurse Practitioner (ANP) to join our dynamic primary care team at Christchurch Medical Practice Purewell. The ANP will work autonomously within their professional scope, providing care through face-to-face consultations, telephone assessments, and home visits when required. This role requires advanced clinical knowledge, critical decision-making skills, and the ability to manage a diverse patient caseload. The successful candidate will provide expert advice, triage and treat patients, coordinate referrals, and ensure the highest standards of clinical safety and excellence. The postholder will be subject to an Enhanced Disclosure and Barring Check (DBS).

Main duties of the job

Clinical Nursing Practice

• Provide advanced clinical care using expert assessment, clinical judgment, triage, same-day ANP support, and management of urgent/unplanned events.
• Diagnose, manage, and treat acute and chronic conditions in adults and children.
• Prescribe and review medications in line with best practice, national guidance, and protocols.
• Request and interpret investigations (e.g., bloods, X-rays, scans) within agreed protocols.
• Refer appropriately within primary/secondary care and liaise with multidisciplinary teams.

Care Management

• Communicate effectively with patients, carers, and families regarding care plans.
• Monitor and support achievement of clinical targets (e.g., QOF, prescribing schemes).
• Ensure care outcomes meet required standards and recommend improvements.

Health Improvement

• Lead screening and prevention initiatives.
• Support innovation in care delivery and evidence impact through reports/presentations.
• Refer to statutory and voluntary services to improve patient outcomes.

Administration & Professional Responsibilities

• Maintain accurate, contemporaneous patient records in line with policy and legislation.
• Participate in meetings, audits, and governance activities.
• Ensure clinical areas are stocked, maintained, and compliant with infection control standards
