Job summary

IC24 in Hastings is seeking an Advanced Nurse Practitioner to lead autonomous clinical care at first contact, diagnosing and treating unregistered patients within a multidisciplinary team.

You will prescribe where appropriate as an Independent Prescriber, support vulnerable groups, and direct patients to suitable care pathways while contributing to governance and service development.

Qualifications

  • Must be a First Level Registered Nurse with current NMC registration.
  • Holds an Independent Prescriber qualification.
  • Has completed a Physical Assessment of Adults module and a recognised Minor Illness qualification.
  • Has at least two years' experience working within General Practice or Urgent Care.

Responsibilities

  • Deliver advanced clinical care at the point of first contact, assessing, diagnosing and treating patients.
  • Prescribe medications as an Independent Prescriber following clinical assessment.
  • Support unregistered patients and those facing health inequalities to improve access to care.
  • Navigate patients to the most appropriate care pathway, including referrals when needed.
  • Drive quality and clinical governance within IC24’s framework and contribute to audits and service development.
  • Work collaboratively with colleagues and external partners to share learning and improve patient care.
  • Develop Hastings Primary Care Hub services and contribute to innovation and better outcomes.

Job description

The Role

Primary care is where advanced clinical skills make the greatest difference.

It's where you assess, diagnose and treat patients at the point of first contact. It's where clinical expertise, autonomy and decision-making come together to deliver safe, effective care for people who may otherwise struggle to access healthcare services.

At IC24, we're looking for an Advanced Nurse Practitioner who wants to use their skills in a role that combines clinical leadership, autonomous practice and the opportunity to improve outcomes for some of the most vulnerable patients in our community.

Based within our Hastings Primary Care Hub, you'll lead on delivering high-quality care to unregistered patients, working as part of a multidisciplinary team to assess, diagnose, treat and navigate patients to the most appropriate care pathway.

The role also involves working closely with local partners, including the Seaview Project, supporting individuals experiencing homelessness, those living in temporary accommodation and patients with complex health and social care needs. This includes people affected by mental ill health, substance misuse, learning disabilities and other barriers to accessing healthcare.

What you'll focus on
  • Delivering advanced clinical care

    You'll provide assessment, diagnosis and treatment at the point of first contact, managing patients with minor illness and undifferentiated presentations while delivering safe, evidence-based care.

  • Prescribing and clinical decision making

    As an Independent Prescriber, you'll prescribe medications following clinical examination and assessment, ensuring treatment plans are appropriate, effective and aligned to current best practice.

  • Supporting vulnerable populations

    You'll work with unregistered patients and those facing significant health inequalities, helping improve access to healthcare and supporting patients with complex physical, psychological and social needs.

  • Navigating patients to the right care

    You'll determine the most appropriate care pathway for each patient, whether that's treatment and discharge, onward referral or navigation into alternative healthcare services.

  • Driving quality and clinical governance

    You'll work within IC24's clinical governance framework, contributing to audits, quality improvement initiatives, service development and the continuous enhancement of patient care.

  • Working collaboratively

    You'll build strong working relationships across the organisation and with external partners, contributing to shared learning, locality meetings and collaborative approaches to patient care.

  • Developing services and supporting innovation

    You'll contribute to the ongoing development of the Hastings Primary Care Hub, helping shape services, share best practice and improve outcomes for patients and local communities.

What matters in this role
