Job summary

Integrated Care 24 (IC24) seeks an Advanced Nurse Practitioner to lead advanced clinical care within Hastings Primary Care Hub. You will assess, diagnose and treat unregistered patients, working with a multidisciplinary team to determine the right care pathway.

The role focuses on autonomous practice, prescribing, and improving access and outcomes for vulnerable groups, including those experiencing homelessness and mental health challenges.

Qualifications

  • Be a First Level Registered Nurse with current NMC registration.
  • Hold an Independent Prescriber qualification.
  • Has completed a Physical Assessment of Adults module and a recognised Minor Illness qualification.
  • Has at least two years' experience in General Practice or Urgent Care.

Responsibilities

  • Provide advanced clinical care including assessment, diagnosis and treatment at first contact.
  • Prescribe medications as an Independent Prescriber following examination and assessment.
  • Support vulnerable populations and improve access to care.
  • Navigate patients to appropriate care pathways and services.
  • Contribute to clinical governance, audits and service development.
  • Collaborate with internal teams and external partners to improve patient care.

Advanced Nurse Practitioner - Primary Care

The Role

Primary care is where advanced clinical skills make the greatest difference.

It's where you assess, diagnose and treat patients at the point of first contact. It's where clinical expertise, autonomy and decision-making come together to deliver safe, effective care for people who may otherwise struggle to access healthcare services.

At IC24, we're looking for an Advanced Nurse Practitioner who wants to use their skills in a role that combines clinical leadership, autonomous practice and the opportunity to improve outcomes for some of the most vulnerable patients in our community.

Based within our Hastings Primary Care Hub, you'll lead on delivering high-quality care to unregistered patients, working as part of a multidisciplinary team to assess, diagnose, treat and navigate patients to the most appropriate care pathway.

The role also involves working closely with local partners, including the Seaview Project, supporting individuals experiencing homelessness, those living in temporary accommodation and patients with complex health and social care needs. This includes people affected by mental ill health, substance misuse, learning disabilities and other barriers to accessing healthcare.

What you'll focus on
Delivering advanced clinical care

You'll provide assessment, diagnosis and treatment at the point of first contact, managing patients with minor illness and undifferentiated presentations while delivering safe, evidence-based care.

Prescribing and clinical decision making

As an Independent Prescriber, you'll prescribe medications following clinical examination and assessment, ensuring treatment plans are appropriate, effective and aligned to current best practice.

Supporting vulnerable populations

You'll work with unregistered patients and those facing significant health inequalities, helping improve access to healthcare and supporting patients with complex physical, psychological and social needs.

Navigating patients to the right care

You'll determine the most appropriate care pathway for each patient, whether that's treatment and discharge, onward referral or navigation into alternative healthcare services.

Driving quality and clinical governance

You'll work within IC24's clinical governance framework, contributing to audits, quality improvement initiatives, service development and the continuous enhancement of patient care.

Working collaboratively

You'll build strong working relationships across the organisation and with external partners, contributing to shared learning, locality meetings and collaborative approaches to patient care.

Developing services and supporting innovation

You'll contribute to the ongoing development of the Hastings Primary Care Hub, helping shape services, share best practice and improve outcomes for patients and local communities.

You're likely someone who:

  • Is a First Level Registered Nurse with current NMC registration
  • Holds an Independent Prescriber qualification
  • Has completed a Physical Assessment of Adults module and a recognised Minor Illness qualification
  • Has at least two years' experience working within General Practice or Urgent Care
  • Demonstrates excellent clinical assessment and decision-making skills
  • Is confident working autonomously and managing clinical risk
  • Has experience of service improvement, audit and clinical governance
  • Communicates effectively with patients, colleagues and partner organisations
  • Is committed to continuous professional development and evidence-based practice
  • Thrives in a dynamic environment and enjoys working collaboratively across services
