An established healthcare provider in Grantham is seeking an Advanced Nurse Practitioner (ANP) to provide specialized nursing care, manage patient needs independently, and enhance community health. This role requires autonomous decision-making, collaboration with a clinical team, and a commitment to high-quality patient care.
The Advanced NursePractitioner (ANP) has been assessed competent in practice using their clinicalknowledge and skills. They have the freedom and authority to act and makeautonomous decisions in the assessment, diagnosis and treatment of patients.
The ANP provides anopportunity for patients to avoid the need to be referred to a GP, therebyenabling them to receive timely care and negate unnecessary delay in receivingtreatment.
ANPs in primary care can develop close, long-term relationships with their patients and work in partnership with them to achieve optimum health. They are autonomous in making decisions based on assessment, diagnosis and interpretation of test results. They can independently prescribe appropriate medication, evaluate and refer to other specialists if necessary.

You will be joining an established clinical team working alongside both our GP's and Nursing team to provide care to our patient population.
We are also a dispensing practice.
Develop,implement and embed health promotion and wellbeing programmes
Managepatients presenting with a range of acute and chronic medical conditions,providing subject matter expert advice
Implementand evaluate individual specialised treatment plans for chronic diseasepatients
Identify,manage and support patients at risk of developing long-term conditions,preventing adverse effects to the patients health
Provideadvanced, specialist nursing care t patients are required in accordance withclinical based evidence, NICE and NSF
Providewound care (ulcer/doppler, etc)
Providespecialist clinics such as contraception etc
Undertakethe collection of pathological specimens
Requestpathology services as necessary
Processand interpret pathology and other test results as required
Providechronic disease clinics, delivering patient care as necessary, referringpatients to secondary/specialist care as required
Maintainaccurate clinical records in conjunction with extant legislation
EnsureSNOMED CT codes are used effectively
Maintainchronic disease registers
Develop,implement and maintain well woman, well man and travel clinics
Take partin implementation of an effective immunisation programme
Prioritisehealth issues and intervene where necessary
Supportthe team in dealing with clinical emergencies
Recognise,assess and refer patients presenting with mental health needs
Implementvaccination programmes for adults and children
Be an extendedand supplementary prescriber, adhering to extant guidance
Supportpatients in the use of their prescribed medicines or over the counter medicines(within own scope of practice), reviewing annually as required
Contributeto practice targets (QOF etc), complying with both local and regional guidance
Liaisewith the external services/agencies to ensure the patient is supportedappropriately (vulnerable patients etc)
Delegateclinical responsibilities where appropriate (ensuring safe practice and thatthe task is within the scope of practice of the individual)
Supportthe clinical team with all safeguarding matters in accordance with local andnational policies
Understandpractice and local policies for substance abuse and addictive behaviour,referring patients appropriately
Deliveropportunistic health promotion where appropriate
