Job summary

Somerset NHS Foundation Trust is seeking an Advanced Clinical Practitioner for the Same Day Emergency Care service within the Emergency Department. The role focuses on assessing and treating adults, deciding care pathways, and discharging or admitting patients promptly.

Postholders may be trainees or qualified, on a level 7 apprenticeship route, with 80% clinical work and 20% supported activities. Registration and MSc/(or commitment) are required.

Qualifications

• BSc in a relevant healthcare profession.
• Agreement to complete an MSc in advanced practice or equivalent to national standard.
• Evidence of continuing professional development.

Responsibilities

• Assess and treat adult patients in the SDEC area of the Emergency Department.
• See undifferentiated and undiagnosed patients across the SDEC scope of practice.
• Decide intervention plans and pathways of care with supervision.
• Balance clinical, social and functional risks to make timely decisions.
• Carry out complex discharges directly from SDEC.
• Identify patients for admission when appropriate.
• Provide holistic, patient-focused care and advocate for patients' needs.
• Promote best practice using national guidelines.
• Spend around 80% of time on clinical work and 20% on supported activities.
