Job summary

Northern Care Alliance NHS Foundation Trust invites applications for an Advanced Clinical Practitioner to join the Same Day Emergency Care service in Oldham. This is a permanent, full‑time role (37.5 hours/week) requiring an MSc in Advanced Clinical Practice.

Responsibilities

Autonomously assessing, investigating and managing same day emergency medical patients will be your primary role; you will in-reach into the Emergency Department (ED) and support referrals from GPs and NWAS.
Your clinical caseload will be diverse and case management should be underpinned by patient-centred care and collaborative working with multi-disciplinary teams to create tailored management plans.
We strive to avoid unnecessary admissions and monitor patients via an ambulatory pathway with sound clinical reasoning and risk management.
