Job summary

Locum Ready is seeking an Advanced Clinical Practitioner to support regional community health and primary care services across Midhurst. This flexible, sessional role suits ACPs with autonomous clinical assessment experience and the ability to manage urgent patient demand.

You will work within established clinical teams, delivering ACP-led sessions, handling acute presentations, and supporting triage by phone or in person. DBS checks required.

Qualifications

• Advanced Clinical Practitioner qualification (Level 7) is required.
• Clinical background in Nursing, Paramedic Science, AHP or similar is required.
• Independent Prescribing (V300) is desirable.
• Experience in GP, urgent care or community health services is essential.
• Ability to work independently in varied clinical environments and Enhanced DBS.

Responsibilities

• Delivering ACP-led sessions within community and primary care settings.
• Managing acute, undifferentiated and minor illness presentations.
• Supporting urgent patient demand through telephone or face-to-face triage.
• Prescribing safely within scope of competence.
• Managing clinical actions including results, medication reviews and follow-up tasks.
• Recording accurate and clear digital documentation.
