Job summary

FCMS, a social enterprise providing health and wellbeing services, seeks an advanced clinical practitioner (nurse prescriber) to join Compass Medical Practice. The role involves autonomous patient management, prescribing under nurse prescriber legislation, and collaboration with a multidisciplinary team to ensure high-quality care.

Responsibilities

  • Autonomously assess, diagnose, plan, implement, and evaluate care for patients.

  • Support and supervise junior staff within a multidisciplinary team.

Job description

Compass Medical Practice is a GP practice for thosewho are difficult to manage in mainstream practice. Patients are eitherconsulted via telephone/ video link or face to face. When a face-to-faceappointment is required, a risk assessment is undertaken to assess what supportthe clinician requires. Consultations are supported with the use of patientliaison officers and consultations are carried out in our mobile clinic van orin an agreed prebooked venue.

Compass Medical Practice also provides medical coverto temporary patients residing at Thornton House. These are short term patientsthat are residing at Thornton house for either rehab, rest or recouperation. Thereis a weekly ward round and multidisciplinary team meeting. Compass Medicalpractice also offers medical support throughout the week, outside of the wardround, during opening hours.

Main duties of the job

We are looking for individuals with a passion andenthusiasm to work with some of the most vulnerable and complex people withinour community. The right candidate should be approachable and display excellentcommunication skills, compassion and a non-judgemental attitude. We are lookingfor an advanced clinical practitioner to join the team who can workautonomously within the special allocation scheme, as well as the widerinclusion health services.

Job responsibilities

Day to Day Duties to include, but notexhaustive:

Practiceautonomously, as an independent practitioner/prescriber, participating in thecare of and undertaking assessment, diagnosis, treatment / referral, anddischarge of patients presenting with minor illness or injury within agreedparameters

Support theManagement team to ensure all QOF Targets are met

Provide support forjunior members of staff

Working closely withthe management team to develop and support new opportunities

Provide advancedclinical skills and advice demonstrating a sound understanding of the issuesrelated to identification, assessment, diagnosis, treatment, and management ofa client group whilst working autonomously

Identify the needfor and initiate and interpret relevant diagnostic investigations and results

To safely andcompetently undertake invasive and non-invasive procedures to establish adiagnosis

To work within theparameters of current nurse prescribing legislation

Monitor and leadimprovements to standards of care through supervision of practice, clinicalaudit, and evidence-based practice, teaching and supporting professionalcolleagues and the provision of skilled professional leadership

Act in accordancewith the NMC Code of Professional Conduct

Provide professionaland clinical advice within the multi-disciplinary team

Produce accurate,contemporaneous, and complete records of patient consultation, consistent withlegislation, policies and procedures

Prioritise,organise, and manage own workload in a manner that maintains and promotesquality

Deliver careaccording to local and national guidelines

Obtain and documentconsent for any treatment undertaken and keep the patient at the centre ofhis/her own care, ensuring full participation in decision making

Work within allrelevant policies and procedural guidelines e.g., infection control,chaperoning and risk management

Safeguardingresponsibilities for adults and vulnerable groups, reporting concernsappropriately at the correct time and engaging with services as needed.

The Head of nursingmay also ask for other duties to be carried out to the benefit of theorganisation.

Clinicalpractice

Autonomously assess,diagnose, plan, implement and evaluate treatment/interventions and care forpatients presenting with an undifferentiated diagnosis and those with long termconditions in line with national and local policies and practice/ network guidelines

Diagnose and manageboth acute and chronic conditions, integrating both drug- and non-drug-basedtreatment methods into a management plan

Prescribe medicationfor therapeutic effectiveness, appropriate to patient needs and in accordancewith evidence-based practice and national and practice protocols, and withinscope of practice

Work with patientsto support concordance with and adherence to prescribed treatments, provideinformation and advice on prescribed or over-the-counter medication onmedication regimens, side-effects, and interactions

Promote health andwell-being using health promotion, health education, screening, and therapeuticcommunication skills

Communicate highlysensitive condition related information to patients, relatives, and carers

When required,ensure patients are referred to other members of the primary health care teame.g., podiatry, or secondary care as necessary

Clinically examineand assess patients needs from a psychological and physiological perspectiveand plan clinical care accordingly

Ensure the correctuse of clinical templates, read codes and protocols

Inform the ClinicalManager /practice manager of any untoward incidents e.g., falls, verbalaggression, criminal damage, safeguarding concerns

Contribute whenrequired to internal and external multi-disciplinary meetings and the decisionmaking of complex vulnerable patients
