Job summary

Herefordshire and Worcestershire Health and Care NHS Trust is seeking an Advanced Clinical Practitioner to support countywide Hospital at Home (H@H) services in Worcestershire. The role emphasises admission prevention with strong respiratory expertise and independent clinical practice.

The post involves working in a developing service, contributing to audits, service development, and collaboration with Acute Trust colleagues. The role requires NMC registration and a full MSc in ACP.

Qualifications

• Registered professional with NMC/HCPC or equivalent.
• Masters in Advanced Clinical Practice.
• Non-Medical Prescribing (V300) and PGD experience.
• Advanced Assessment Skills Module.
• Portfolio of ongoing professional development.

Responsibilities

• Assess and manage acutely unwell patients in the community with medical support.
• Provide urgent care, treat minor illness, give patient advice and manage chronic disease.
• Lead in management of respiratory caseload and educate the H@H team.
• Travel across Worcestershire county as required.
