Job summary

Herefordshire and Worcestershire Health and Care NHS Trust is seeking an enthusiastic ACP to deliver advanced assessment, clinical decision-making and treatment to adults with complex, urgent and changing healthcare needs in the Evening & Night Community Nursing Service, Worcestershire, based at Droitwich.

You will undertake advanced clinical assessments, provide leadership to colleagues, and work evenings and nights to support admission avoidance and safe care closer to home, with opportunities

• Prescribe within scope using appropriate PGDs and develop home care plans.
• Provide clinical leadership, education and supervision to colleagues.

Responsibilities

• Undertake advanced clinical assessments, physical examinations and diagnostic reasoning for complex and urgent presentations.
• Prescribe within scope and use PGDs; interpret investigations and develop treatment plans for home care.
• Assess patients at risk of hospital admission and support safe escalation or transfer of care.
• Provide clinical advice to community nursing colleagues and collaborate with GPs and ambulance services.
• Initiate and review ReSPECT documentation where appropriate and support service improvement.

"What if your advanced clinical assessment could prevent a patient from spending the night in hospital?" Join our countywide Evening & Night Community Nursing Service in Worcestershire, currently based at Droitwich. We are seeking an enthusiastic, innovative and clinically confident ACP to deliver advanced assessment, clinical decision-making and treatment to adults with complex, urgent and changing healthcare needs. Working evenings and nights alongside experienced community nursing teams and urgent and emergency care partners, you will support admission prevention, ambulance avoidance and safe care closer to home. You will also attend some daytime service meetings and build strong working relationships with Hospital at Home, UCR and Neighbourhood Teams. We are committed to outstanding patient care and offer flexibility, professional development, wellbeing support and opportunities for long-term career progression., As our ACP, you will provide senior clinical expertise and leadership within the Evening and Night Service, supporting nurses and wider teams to safely manage increasingly complex patients in their own homes. You will: Undertake advanced clinical assessments, physical examinations and diagnostic reasoning for complex and urgent presentations. Prescribe within your scope and use appropriate PGDs, request and interpret investigations, and develop treatment plans to support safe care at home. Assess and manage patients at risk of hospital admission, supporting admission avoidance and safe escalation or transfer of care. Provide expert clinical advice to community nursing colleagues and work collaboratively with GPs, urgent community response, ambulance, acute and wider health and social care services. Initiate and review ReSPECT documentation where appropriate. Provide clinical leadership, education, supervision and professional support to colleagues. Contribute to service improvement, innovation, audit and research, ensuring opportunities support the four pillars of advanced practice alongside clinical responsibilities.
