Job description

The post holder will apply advanced clinical expertise, levels of judgement, discretion and decision making to the provision of clinical care of people living with frailty within a variety of healthcare settings. They will exercise a high degree of autonomy through the application of advanced clinical skills in relation to the assessment, diagnosis, treatment and management of the person living with frailty who may be acutely unwell, recovering from illness or injury or in need of proactive assessment.

The post holder will lead and monitor improvements to quality of care through clinical audit, evidence-based practice, clinical supervision, teaching and supporting professional colleagues and the provision of skilled professional leadership. They will lead the implementation of relevant clinical guidelines into practice and contribute to the further development of the evidence base through research and audit.

The post holder will lead the development of multidisciplinary models of care across inpatient wards, urgent care, care homes and be part of the integrated care system. They will play a crucial role in improving choice and quality of life for patients through a patient centred approach to timely assessment, referral, responsive intervention, anticipatory care planning and optimal discharge coordination across seven days.

This role is based within the community, as such applicants must hold relevant UK driver’s licence or be working towards obtaining one.

The role requires an advanced level of responsibility and autonomy to make professionally accountable decisions, including differential diagnosis, prescribing medication and delivery of care, often from referral to discharge in unpredictable situations. As an Advanced Clinical Practitioner (ACP’s) you are empowered to use your advanced knowledge, skills and judgement for high-level and complex decision-making in an expanded scope of practice role.

This role involves delivering advanced clinical practice, supporting service development, and fostering innovation. Responsibilities include designing and implementing integrated care programs, influencing decision-makers, driving business objectives, and strengthening cross-organisational networks. You'll provide expert clinical guidance, promote high-quality, evidence-based care, and oversee service frameworks, protocols, and standards.

Patient safety is central, requiring risk management, safeguarding, compliance monitoring, and incident reporting. You'll contribute to effective service delivery through policy influence, advanced clinical assessments, and workforce development while leading research, audits, and service improvements.

Patient experience is enhanced through compassionate, patient-centered care, effective communication, and strategic partnerships. You'll mentor colleagues, support learning initiatives, engage in leadership, and facilitate continuous professional development.
