At IC24, we’re looking for an Advanced Clinical Practitioner (ACP) to join our East Sussex Home Visiting Service. This is an opportunity to work at the forefront of community-based care, delivering high-quality clinical assessments and treatment to patients in their own homes while supporting wider urgent and primary care services.
As an experienced clinician, you'll work autonomously within your professional scope of practice, providing assessment, diagnosis, treatment and prescribing for patients presenting with acute, urgent and undifferentiated conditions. You'll play a key role in helping patients receive the right care in the most appropriate setting, reducing unnecessary hospital admissions and improving patient outcomes.
You’ll assess, diagnose and manage patients in their own homes, providing high-quality clinical care for a wide range of urgent and primary care presentations.
You’ll use advanced clinical reasoning skills to determine appropriate treatment plans, referrals, investigations and follow-up care while managing clinical risk effectively.
You’ll work closely with GPs, nurses, allied health professionals, NHS 111, community services and secondary care partners to ensure patients receive coordinated and seamless care.
You’ll support audits, clinical governance activities, shared learning and quality improvement initiatives that enhance patient outcomes and service delivery.
You’ll maintain your continuing professional development, contribute to mentoring and shared learning, and help promote best practice across the wider organisation.

Due to the nature of this position, employment is subject to proof of eligibility to work in the UK, completion of a satisfactory enhanced DBS disclosure and three years of references.
