Weare looking for a full or part time dedicated and skilled AdvancedClinical/Nurse Practitioner to join our supportive and friendly healthcareteam. The role is initially to cover Maternity Leave but may well lead on to apermanent position in the team.
The successful candidate will play a pivotal role indelivering high-quality patient care, working collaboratively with otherhealthcare professionals to assess, diagnose, and manage patient conditions.This position requires a strong clinical background and the ability to workautonomously while also contributing to a multidisciplinary team.
Practice in compliance with respective code of professional conduct and within their scope of practice,being responsible and accountable for their decisions, actions and omissions
Demonstrate a critical understandingof their broadened level of responsibility and autonomy and the limits of owncompetence and professional scope of practice
Demonstrate effective communicationskills, supporting people in making decisions, planning care or seeking to makea positive change
Use expertise and decision-making skills to informclinical reasoning
Initiate, evaluate and modify a range of interventionswhich may include prescribing medicines, therapies, lifestyle advice and care
Manage patients presentingwith a range of acute and chronic medical conditions, providing subject matterexpert advice
Implement and evaluateindividual specialised treatment plans for chronic disease patients
Identify, manage and supportpatients at risk of developing long-term conditions, preventing adverse effectsto the patients health
Actively engage in peer review to inform own practice, formulating and implementing strategies to act on learning and makeimprovements
Provide chronic disease clinics,delivering patient care as necessary, referring patients tosecondary/specialist care as required
Maintain accurate clinical records inconjunction with extant legislation
Contribute to practice targets (QOF, etc.)
Support the team in dealing withclinical emergencies
A little about the surgery and its philosophy andperformance
Francis Grove Surgery is an established GP practice in theheart of Wimbledon. The practice is a 3 partner practice, with a list size of14,700 patients. It is situated in modern premises on the ground, first andsecond floor in the Courtyard Development only minutes walk away fromstations, shops, cafes and Wimbledon Shopping Centre.
Wimbledon is a vibrant and popular suburb of London withexcellent links to other parts of London via underground and overgroundstations, buses and trams. Much of the area is made up of comfortable privatehousing with very little social deprivation. The practice population is wellinformed, comprising a large number of working professionals and their youngfamilies.
Francis Grove Surgery has an excellent reputation locallyand is a popular practice. Feedback from patients is very good and they tend tostay with the practice.

Thefollowing are the core responsibilities of the Advanced Clinical Practitioner.
There may be, on occasion, a requirement tocarry out other tasks; this will be dependent upon factors such as workload andstaffing levels:
a.Practise in compliance with their respectivecode of professional conduct and within their scope of practice, beingresponsible and accountable for their decisions, actions and omissions at thislevel of practice
b.Demonstrate a critical understanding of theirbroadened level of responsibility and autonomy and the limits of own competenceand professional scope of practice, including when working with complexity,risk, uncertainty and incomplete information
c.Act on professional judgement about when toseek help, demonstrating critical reflectionon own practice, self-awareness, emotional intelligence, and openness to change

h.Exercise professional judgement to managerisk
i.Managepatients presenting with a range of acute and chronic medical conditions,providing subject matter expert advice
j.Implementand evaluate individual specialised treatment plans for chronic diseasepatients
k.Identify,manage and support patients at risk of developing long-term conditions,preventing adverse effects to the patients health
l. Be a professional role model for the organisation
m. Actively engage in peer review to inform own and otherspractice, formulating and implementing strategies to act on learning and makeimprovements
n. Actively seek feedback and involvement from individuals,families, carers, communities and colleagues in the co-production of serviceimprovements
o. Critically assess and address own learning needs,negotiating a personal development plan that reflects the breadth of ongoingprofessional development across the four pillars of advanced clinical practice(clinical practice, leadership and management, education and research).
p.Providechronic disease clinics, delivering patient care as necessary, referringpatients to secondary/specialist care as required
q.Maintainaccurate clinical records in conjunction with extant legislationand to ensure SNOMED CT codes are used effectively
r.Develop,implement and embed an effective call/recall system
s. Critically appraise and synthesise the outcome ofrelevant research, evaluation and audit, using the results to underpin ownpractice and to inform that of others
t.Develop andimplement robust governance systems andcontribute to practice targets (QOF, etc.), complying with local and regionalguidance
u.Chaperonepatients where necessary
v.Supportthe team in dealing with clinical emergencies
w.Supportthe team with all safeguarding matters in accordance with local and nationalpolicies
x.Understandpractice and local policies for substance abuse and addictive behaviour,referring patients appropriately
y.Deliveropportunistic health promotion where appropriate
Inaddition to the primary responsibilities, the Advanced Clinical Practitionermay be requested to:
a.Supportthe IPC lead, ensuring compliance, and undertaking audit as necessary
b.Monitorand ensure the safe storage, rotation and disposal of medicines
c.Undertakeoverall management of a clinical team, providing guidance when necessary,acting as a mentor to students and newly qualified staff
d.Maintainan effective clinical staff rota ensuring all clinics are staffed appropriately
e.Participatein local initiatives to enhance service delivery and patient care
f.Supportand participate in shared learning within the organisation
g.Developan area of specialist interest, taking the lead within the organisation
h.Continuallyreview clinical practices, responding to national policies and initiativeswhere appropriate
i.Participatein the review of significant and near-miss events applying a structuredapproach i.e., root cause analysis (RCA)
j.Commensuratewith their role, support the development of clinical services within theorganisation, liaising with external agencies and professional organisations asrequired
Develop practice administrative and clinicalprotocols in line with the needs of the patient and current legislation
