Job summary

Health Care First Partnership in West Yorkshire seeks an experienced Advanced Clinical Practitioner to lead clinical services and drive quality improvement within primary care. You will work across community, secondary care interfaces and mentor teams to deliver high standards of patient care.

The role requires current UK registration, a postgraduate qualification in Advanced Clinical Practice, and strong leadership, teaching and governance capabilities.

Qualifications

• Undergraduate nursing degree required for registration and practice.
• Postgraduate Diploma in Advanced Clinical Practice with digital badge/RCN credentials.
• Postgraduate Masters in Advanced Clinical Practice is desirable.
• Mentor/teaching qualification and clinical supervision training preferred.
• Current UK NMC registration, including status as independent prescriber.
