Advance Nurse Practitioner/Paramedic Practitioner - New Horizons Medical Partnership
The duties and responsibilities for members of the urgent care team are varied and will evolve based on current practice needs, under the guidance of the Lead Clinical Practitioner, Partners, or Practice Manager:
- Work autonomously to provide nursing care within the GP surgery, patients' homes, and nursing or residential homes.
- Maintain patient confidentiality at all times.
- Assess emergencies, plan, implement, and evaluate acute medical problems, both in person and via eConsults or telephone.
- Respond to emergencies in the surgery, including ECGs.
- Assess, diagnose, and treat conditions independently or as part of the Urgent Care Team, following protocols.
- Take comprehensive histories and examinations for specified conditions to enable provisional diagnoses.
- Prioritize health issues and intervene appropriately in urgent or emergency situations, initiating emergency care when needed.
- Triages patient problems to determine the necessity of face-to-face consultations and appropriate clinician referral.
- Develop and communicate treatment plans effectively to patients and colleagues.
- Conduct annual health reviews for patients with chronic conditions or epilepsy, as required.
- Liaise with other professionals and services to ensure patient-centered care.
- Perform opportunistic screenings such as blood pressure, BMI, and smoking status.
- Advise on family planning and provide contraception services, if appropriate.
- Be competent in ECG, anaphylaxis management, and resuscitation techniques.
- Have a basic understanding of asthma management and nebulization.
- Recognize signs of child abuse and be aware of the Child Protection register.
- Promote health and lifestyle modifications.
- Follow infection control procedures.
- Input data into the practice's IT system.
- Assist with clinical audits.
- Maintain NMC/HCPC registration at all times.
- Contribute to QOF requirements, using spare appointments for QOF-related work.
- Report errors or complaints following practice guidelines.
- Teach and mentor other staff members.
- Perform general nursing tasks as needed and other tasks suitable for the role.
- Cover PAN PCN duties as required.

This is a part-time position, requiring a minimum of 17 hours per week, working Thursdays and Fridays, plus additional days as needed.
