Qualifications

• Qualified Social Worker with current Social Work England registration.
• Significant post-qualification experience in Adult Social Care.
• Experience in one or more specialist adult social care service areas.
• Strong experience in assessment, risk management, safeguarding and care planning.
• Solid knowledge of Care Act 2014, Mental Capacity Act and safeguarding procedures.
• Ability to prioritise and work independently within a busy environment.
• Excellent verbal and written communication and partnership-working skills.

Responsibilities

• Undertake strengths-based assessments and develop person-centred care and support plans.
• Manage a complex caseload while ensuring statutory duties and legislative requirements.
• Complete Mental Capacity Act assessments and Best Interest decisions where appropriate.
• Conduct reviews, reassessments, and safeguarding enquiries in accordance with local authority procedures.
• Work collaboratively with service users, carers, families, health professionals, and partner agencies.
• Prepare high-quality reports, assessments, and documentation, including court reports where required.
• Support adults to maximise independence, wellbeing, and community participation.
• Undertake risk assessments and implement effective risk management strategies.
• Contribute to service improvement initiatives and the ongoing development of Adult Social Care services.
• Promote strengths-based, outcome-focused, and person-centred approaches to practice.
