Job Description:

Key Responsibilities:

  • Provide local and personal social care services for vulnerable customers, contributing to voluntary and statutory social work activities.
  • Conduct investigations and assessments, producing reports for complex cases.
  • Assess customer needs in line with council policies and legislation, including joint assessments, developing intervention strategies, and support services.
  • Develop support plans, coordinate services, and manage care packages within financial constraints.
  • Monitor and review support plans to ensure standards and best practices are met.
  • Promote principles of prevention, reduction, and delay, ensuring dignity and respect for service users.
  • Offer information, advice, and guidance on local services and support options.
  • Provide ongoing support to clients and carers, acting as case holder where applicable.
  • Maintain accurate records using departmental IT systems.
  • Collaborate with other organizations to ensure service continuity.
  • Participate in developing preventative strategies and support services.
  • Conduct safeguarding investigations.
  • Undertake additional training such as Practice Educator, subject to eligibility.

Minimum Requirements:

  • A professional qualification in social work.
  • Social Work England registration.
  • Enhanced DBS/Update service.
  • References for the last 5 years of employment.
