Job summary

Remedy Social Work is seeking an Adults Social Worker to join the Learning Disabilities & Autism team in Leicestershire. You will deliver high quality, person-centred social care interventions and coordinate assessments for adults with complex needs.

Successful candidates will work in partnership with health and other agencies to promote independence, manage safeguarding, and deliver outcome-focused care plans within statutory frameworks.

Qualifications

• Deliver high quality, person-centred social care service.
• Assess, manage and review needs and risks for adults.
• Support independence and self-directed responses.

Responsibilities

• Provide appropriate responses or referrals within required timescales, policy and legislative requirements.
• Provide prompt support to meet immediate risks and needs in crisis situations.
• Support citizens' self-assessments of their needs in partnership with health and other agencies.
• Act as care co-ordinator for citizens with complex needs and high-risk situations.
• Ensure responses consider citizens' ethnic and religious needs.
• Identify, investigate and manage safeguarding, mental capacity and other high-risk situations.
