Ref: 001825

Contract: Fixed Term – 3 years

Salary: £32,578 - £35,570 per annum (Grade 5)

Hours: 36.66 per week - hybrid working

Location: Oldham Council, Spindles Shopping Centre, George Street, Oldham, OL1 1HD

Closing Date: Thursday 12 November 2026

Interview Date: TBC.

Role Overview:

This is an exciting opportunity to join Oldham’s Changing Futures service as part of the borough’s delivery of the Enhanced Live Well offer for adults experiencing multiple disadvantage.

As a Changing Futures Support Worker, you will work as part of a multidisciplinary, trauma‑informed team, providing intensive, relationship‑based support to adults who experience some of the most complex challenges, including homelessness, substance misuse, mental ill‑health, domestic abuse, and contact with the criminal justice system.

You will play a key role in delivering a person‑centred, outreach‑based approach, supporting individuals who often struggle to engage with traditional services and who may have experienced repeated exclusion from support systems.

Working within Adult Social Care’s Front Door, you will be part of an integrated pathway that includes:

  • Access to community‑based Live Well offers
  • Step‑up into the MEAM Hub (permanent MDT)
  • Allocation to a dedicated Key Worker for ongoing support
  • Contribution to system learning and service improvement

You will take responsibility for case managing a small number of individuals with high levels of complexity, ensuring they receive coordinated, flexible and consistent support that is built around their strengths, goals and aspirations rather than service boundaries.

Key Responsibilities:
  • Provide intensive, outreach‑based support to adults experiencing multiple disadvantage, working flexibly across community settings, homes, and partner organisations
  • Build trusting, consistent relationships with individuals who may be disengaged from services, using a trauma‑informed and strengths‑based approach
  • Lead on and contribute to Team Around the Adult (TAA) meetings, ensuring coordinated multi‑agency support
  • Work in partnership with the MEAM Hub MDT, contributing to shared decision‑making, risk management and planning
  • Develop and review person‑centred support plans, focused on individual goals and meaningful outcomes
  • Advocate on behalf of individuals to challenge barriers within systems, ensuring access to appropriate services
  • Work closely with partners across Adult Social Care, health, housing, substance misuse, VCSE and criminal justice services
  • Identify and elevate risk and safeguarding concerns, ensuring appropriate action is taken in line with policy
  • Support individuals through periods of instability while maintaining engagement and continuity
  • Contribute to the identification of system barriers, service gaps and patterns of disengagement, feeding into service development and improvement
