Job summary

Cambridgeshire County Council is seeking an Adult Support Coordinator to join the Adults Community Team in Huntingdonshire, based in Huntingdon. The role focuses on helping adults live independently and achieve positive outcomes through integrated care and strong partnerships with health colleagues and partner agencies.

You will perform care assessments, develop person-centred plans, manage budgets, and maintain records, within a supportive, flexible working environment that values development

Responsibilities

• Undertaking Care Act and Mental Capacity Act assessments.
• Completing reviews of existing care and support arrangements.
• Developing person-centred care and support plans.
• Working with health professionals and partner agencies to achieve outcomes.
• Undertaking safeguarding inquiries and promoting choice and control.
• Maintaining accurate records and documentation.
• Managing and monitoring allocated budgets and packages.
• Building partnerships to ensure coordinated support.
