Job summary

Recruitment Hub is seeking an Adult Support Coordinator to join the Adults Community Team in Huntingdonshire, based in Huntingdon. You will support adults to live independently and achieve positive outcomes, working with health professionals and partners to deliver person-centred care.

Responsibilities

• Undertaking Care Act and Mental Capacity Act assessments.
• Completing reviews of existing care and support arrangements to meet needs and outcomes.
• Developing person-centred care and support plans that promote independence and wellbeing.
• Collaborating with health professionals and partner agencies to achieve positive outcomes.
• Undertaking safeguarding enquiries and promoting choice and control.
• Maintaining accurate, evidence-based records and documentation.
• Managing and monitoring allocated budgets and support packages.
