Grafham Water Centre in Huntingdonshire is seeking an Adult Support Coordinator to join the Adults Community Team. The role focuses on helping adults live independently, building skills and accessing appropriate services within a supportive locality setting.

We are looking for an Adult Support Coordinator to join our Adults Community Team in Huntingdonshire, based in Huntingdon. What will you be doing?

As an Adult Support Coordinator, you will play a key role in supporting adults to live as independently as possible and achieve the outcomes that are important to them.

Your responsibilities will include:

• Undertaking Care Act and Mental Capacity Act assessments.


• Completing reviews of existing care and support arrangements to ensure they continue to meet individuals' needs and deliver positive outcomes.


• Developing person-centred care and support plans that promote independence and wellbeing.


• Working collaboratively with colleagues, health professionals, and partner agencies to achieve positive outcomes.


• Undertaking safeguarding enquiries and helping to keep people safe whilst promoting choice and control.


• Maintaining accurate, evidence-based records and documentation.


• Managing and monitoring allocated budgets and support packages.


• Building strong partnerships with a range of organisations to ensure coordinated and effective support.

• Show resilience, adaptability, and professionalism in a fast-paced social care setting.


• Have an appreciation of working within budget limitations and balancing available resources with individuals' needs and desired outcomes.
