An exciting opportunity has arisen for an experienced Social Work Practitioner to join Salford’s Adult Health and Social Care Contact Team.
The Adult Health and Social Care Contact Team is the first point of contact for residents of Salford who may need access to health or social care services. In this role, you will work closely with colleagues to provide information, advice, and support to adults and their carers, who may be eligible for assessment and care services under the Care Act 2014.
We aim to support people to live well and safely at home by having strength-based conversations to promote wellbeing. We collaborate with other organizations to ensure individuals are well connected with their local communities and can work towards achieving their personal outcomes.
The Adult Social Care workforce strives to improve outcomes for residents by developing a skilled and motivated team. We invest in ongoing professional development and ensure social work practices are current and of high standard. Candidates should hold a degree in social work, be registered with Social Work England, demonstrate ongoing professional development, and possess a strong understanding of the Care Act, Mental Capacity Act, and Safeguarding Adults.
The main responsibilities include conducting strengths-based assessments and promoting wellbeing by utilizing resources from social care, health, and voluntary or statutory agencies.
The role involves initial assessments, working with complex and legal issues, crisis planning, providing information, advice, signposting, and transferring cases to other teams or services as necessary.
You will need to identify whether further assessment under sections 9 and 10 of the Care Act is required and refer cases to appropriate social work teams for these assessments. You will also handle support planning and reviews.
There is a Safeguarding component, which includes initial information gathering, risk assessment, safety planning, and determining the need for Safeguarding Adult Enquiries under section 42 of the Care Act.
The role demands resilient practitioners capable of managing competing demands and prioritizing effectively. Applicants should be confident decision-makers who can remain calm under pressure.
