Please be aware this is a Long Term Temporary post to start ASAP.

The following is essential criteria:

• Degree in Social Work
• SWE Registration
• Right to work within the UK

Social worker required for hospital discharge assessments in the south team. Responsibilities include reviewing care needs for citizens upon discharge, completing Care Act and Mental Capacity assessments where required, supporting pathways out of hospital, reviewing care packages in the community, supporting discharges from intermediate care beds, and reviewing citizens discharged into pathway 3 settings such as residential and nursing homes.

You will also support crisis response initiatives to prevent hospital admissions and contribute to our hospital at home service.

Must have:

• A professional qualification in social work
• Social Work England registration
• Enhanced DBS/Update service
• References for the last 5 years of employment
