Please be aware this is a Long Term Temporary post to start ASAP.

The following is essential criteria:

• Degree in Social Work
• SWE Registration
• Right to work within the UK

Social worker required for hospital discharge assessments for the south team. Responsibilities include reviewing care needs for citizens on discharge from hospital, completing Care Act assessments and Mental Capacity assessments where required, supporting pathways out of hospital, reviewing care packages in the community, supporting discharges from intermediate care beds, and reviewing citizens discharged into pathway 3 settings such as residential and nursing beds.

Additionally, support crisis response to prevent hospital admissions and assist with the hospital at home service.

You must have:

• A professional qualification in social work
• Social Work England registration
• Enhanced DBS/Update service
• References for the last 5 years of employment
