A leading company in the health sector is seeking a Social Worker for a Long Term Temporary position in Liverpool. The role involves assessing needs under the Care Act, developing support plans, and conducting safeguarding inquiries. Ideal candidates will have a Social Work degree and SWE registration. The role involves attending ward reviews as part of mental health multidisciplinary teams in adult and older persons wards to assess needs under the Care Act and develop strengths-based support plans. The post holder will be key in developing and leading discharge plans, completing Mental Capacity Assessments, and making Best Interest decisions when required. Partnership working with health, housing, children’s services, and other agencies is essential.
The candidate must be a strong communicator with a robust understanding of the Care Act 2014, the Mental Capacity Act 2005, and the Mental Health Act 1983 in practice. Responsibilities include conducting section 117 discharge planning meetings, preparing Social Circumstance Reports for mental health tribunals, and conducting section 42 safeguarding enquiries as needed. The team covers Merseycare wards across the region, currently based in Broadoak Unit, Liverpool.
