Eden Brown Synergy is currently recruiting an experienced Qualified Social Worker (QSW) to join the Adult Social Care Review Team in Gloucestershire. You will undertake focused social care reviews within the Supported Accommodation Service and work as part of a multidisciplinary team to ensure services meet needs and promote independence.

The role is based in Gloucestershire on a temporary three-month contract with potential extension, paying £35 per hour plus a £150 weekly

Qualificações

• Qualified Social Worker with a recognised Social Work qualification.
• Current registration with Social Work England.
• Significant experience within Adult Social Care.
• Experience undertaking social care assessments and reviews.
• Excellent communication, assessment and report-writing skills.

Responsabilidades

• Undertake focused and timely social care reviews for individuals within the Supported Accommodation Service.
• Review existing care and support arrangements to ensure they remain appropriate and outcome-focused.
• Work collaboratively as part of a multidisciplinary team (MDT).
• Engage with service users, families, carers and relevant professionals to understand individual needs and circumstances.
• Identify changes in care and support requirements and recommend appropriate interventions.
• Apply professional judgement when reviewing care packages and making recommendations.
• Ensure reviews are person-centred and promote independence, choice and positive outcomes.
• Maintain accurate and up-to-date case records and documentation.
• Communicate effectively with partner agencies and relevant stakeholders.
• Demonstrate flexibility and adaptability in responding to service requirements.
