Halton Borough Council's Adult Social Care service is seeking a qualified social worker or occupational therapist to support adults over 18, including older people and those with mental health needs. The role is 37 hours and requires a full driving licence.

You will work across teams such as Intermediate Care, hospital, safeguarding, OT, and complex care, delivering person-centred plans and working with NHS colleagues to promote independence.

Qualifications

• Recognised professional qualification such as a Social Work degree, Occupational Therapist degree, MA, Dip SW or equivalent.

Responsabilités

• Provide short- and long-term person-centred support to adults over 18
• Carry out asset-based care planning
• Arrange equipment provision as part of care planning
• Support service users and their carers to promote independence
• Challenge discrimination in day-to-day practice
• Work with adults with complex mental health needs, meeting Care Act criteria and taking a recovery-focused approach
• Work with the Mental Health Trust to provide a responsive, holistic service
• Work in partnership with NHS and community health colleagues, including CPNs, district nurses, physiotherapy and GP practices
• Work with housing, equipment and independent living services, including the Home Improvement Agency, Joint Equipment provision and the Independent Living Centre
• Work with other council services, including Children and Family Services and the Positive Behaviour Support Service
• Work across teams covering intermediate care, hospital, safeguarding, initial assessment, occupational therapy and complex care
