Job summary

Connect2Halton is recruiting an experienced Social Worker to join the Complex Care Team in the Runcorn area, supporting the backlog and ongoing case management.

The role requires ASYE completion, SW England registration, and solid knowledge of Care Act, MCA, and safeguarding. You will work independently to manage a varied caseload and collaborate with MDTs.

Qualifications

• ASYE completed with Social Work England registration.
• Experience in Adult Social Care / Complex Care.
• Knowledge of Care Act, Mental Capacity Act, safeguarding

Responsibilities

• Complete Care Act Assessments
• Undertake outcome-focused reviews
• Perform Mental Capacity Assessments and Best Interest Decisions
• Support safeguarding enquiries under Section 42
• Complete CHC checklists and attend DST meetings
• Participate in duty rota responsibilities
• Collaborate in MDT with internal and external partners

This is an excellent opportunity for a qualified and confident practitioner who has successfully completed their ASYE programme and has experience working within adult social care and complex case management.

Key Responsibilities

• Completing Care Act Assessments
• Undertaking outcome-focused reviews
• Carrying out Mental Capacity Assessments and Best Interest Decisions
• Supporting with safeguarding enquiries under Section 42
• Completing CHC checklists and attending DST meetings
• Participating in duty rota responsibilities
• Working collaboratively within a multi-disciplinary team (MDT) approach alongside internal services and external partner agencies

Requirements

• Qualified Social Worker with Social Work England registration
• Completion of ASYE programme
• Previous experience within Adult Social Care / Complex Care
• Strong knowledge of the Care Act, Mental Capacity Act, and safeguarding legislation
• Ability to work independently and manage a varied caseload
