• Coordinate effectively with health, housing, voluntary-sector and other multidisciplinary services.
• Maintain accurate records and progress casework in line with statutory duties, policy and agreed timescales.
• Promote choice, control, independence and the least restrictive approach throughout practice.

The successful candidate will join an established local-authority service and work with colleagues, individuals, families, carers and partner organisations as appropriate to the assignment. The emphasis will be on timely, well-reasoned practice, clear communication and achieving safe, person-centred outcomes.

Service information and working arrangements

On site SW post at based at Wellington House Assessments in persons home or in a place. The post will have a focus on Care Act assessments, safeguarding, MCA’s, Reviews and Domestic DoLS
