Key attributes/experience:

  • Hands-on experience with Physical Disabilities and Older Persons
  • Experience in substance misuse, domestic violence, safeguarding, homelessness, hoarding, and challenging behaviors
  • Effective communication and collaboration in a Multi-Disciplinary Team (MDT)
  • Familiarity with in-house reablement services
  • Compliance with the Care Act, MCA, and DoLS
  • Advocacy for service users and delivery of person-centred care
  • Assessment and management of complex cases while upholding professional standards,
