Job summary

Leaders in Care is recruiting a Social Worker to join a supportive Adult Front Door/Access service in the South East. This locum contract offers flexible and hybrid working, 36 hours per week, and an opportunity to help adults access the right support while receiving consistent guidance.

You will assess, plan and coordinate care, working with colleagues across social care, health, and housing to promote independence and positive outcomes in adults with complex needs.

Qualifications

• Social Worker qualification required.
• Current Social Work England registration.
• Experience in Adult Social Care and related legislation beneficial.

Responsibilities

• Carrying out comprehensive assessments.
• Creating person-centred support plans reflecting individual needs and goals.
• Managing a varied caseload with complex needs, safeguarding and risk.
• Supporting adults with mental health needs, learning disabilities, autism and other complexities.
• Applying Care Act 2014, Mental Capacity Act 2005 and safeguarding procedures.
• Collaborating with partner agencies to deliver coordinated support.

You will work alongside professionals across social care, health, housing and community services to help adults access the right support, retain their independence and achieve positive outcomes.

What You’ll Be Doing

• Completing comprehensive, strengths-based assessments.

• Supporting adults with mental health needs, learning disabilities, autism and other complex circumstances.

• Promoting independence, choice and wellbeing for the adults you support.

What We’re Looking For

• A recognised Social Work qualification.
• Current Social Work England registration.
• Previous experience in Adult Social Care.
• Strong working knowledge of the Care Act 2014, Mental Capacity Act 2005 and adult safeguarding procedures.
