Royal United Hospitals Bath NHS Foundation Trust in Bath is seeking an activities coordinator to support ward discharge processes. The role is internal only within the BSW group and aims to engage patients to promote independence and confidence as they return home.

You will shape the future of this new position, coordinating with the ward team and external groups to provide meaningful activities and facilitate smoother patient flow and discharge planning.

Formación

• NVQ Level 3 in Health and Social Care or equivalent clinical experience.
• Care Certificate or commitment to complete on joining.

• Engage in one-to-one activities as needed.
• Work with MDT to support completion of 'what matters to me' document.
• Liaise with external groups for music sessions and PAT dogs.

Job Overview

This role is internal only for employees of the BSW group only (RUH Bath, SFT Salisbury, GWH Swindon). You will be assisting with the activities that help facilitate timely and effective discharge from the ward. Within this role you will be proactive and use your creative skills to help deliver the best outcomes for patients by getting them involved in a wide range of activities that will encourage independence and confidence for patients who will be returning to the own place of residence. As this is a new role you will have the chance to shape and direct the future of this position within the ward. Main duties of the job

• Organise patients into dayroom for a range of activities.
• Encourage patients to attend lunch club daily.
• Engage in one to one activities where necessary.

Qualifications

Essential criteria

• NVQ Level 3 in Health and Social Care, or equivalent relevant clinical experience
• Care Certificate (or commitment to complete upon joining)

Desirable criteria

• Formal training in rehabilitation, occupational therapy support, or safe moving and handling techniques

Experience

Essential criteria

• Proven experience working within an NHS acute, community, or social care setting.
• Background in collaborating within a multi-professional team environment.

Desirable criteria

• Experience working explicitly within patient flow, discharge planning, or community outreach models.

Skills and Attributes

Essential criteria

• Exceptional interpersonal and defusing communication skills.
• High-level organization with ability to prioritize under pressure.
• Confident IT literacy for inputting patient health data.

Desirable criteria

• In-depth understanding of local health and social care pathways, including the Home First model.
