Job summary

London Care, part of the CCH Group, is seeking a Health and Wellbeing Coordinator to plan and deliver engaging exercise and cognitive stimulation therapy sessions for older adults. You will motivate clients to maintain health and fitness, build relationships with care teams, and support dementia initiatives while adhering to safety and quality standards.

Responsibilities

  • Be an active member of the Care Team.
  • Plan and deliver group exercise classes for older people, including wellbeing initiatives such as workshops and signposting to wellbeing practitioners.
  • Support the Dementia Project by planning and coordinating cognitive stimulation therapy sessions and peer support for those living with dementia.
  • Design and adapt class content to suit participants’ abilities.
  • Promote awareness of health improvement for older adults.
  • Provide support, motivation, and encouragement to help clients achieve their goals.
  • Maintain client records and monitor progress.
  • Build relationships with clients and support them safely and effectively in all sessions.
  • Ensure fitness equipment is safe and used correctly.
  • Undertake necessary risk assessments.
  • Observe and implement organisation policies and procedures throughout service delivery.
  • Provide first aid when required.
  • Carry out any other duties delegated by the Branch Manager.

Job Description

The Health and Wellbeing Coordinator plans and delivers engaging exercise and cognitive stimulation therapy sessions to improve participants’ physical and mental wellbeing. The role motivates older adults to maintain and improve their health and fitness.

Accountable for the wellbeing of the Service User/Client, you will form relationships with internal stakeholders and external professionals such as commission authority representatives, regulatory inspectors, relatives, advocates and healthcare providers.

Key Duties/Responsibilities
  • Be an active member of the Care Team.
  • Plan and deliver group exercise classes for older people, including wellbeing initiatives such as workshops and signposting to wellbeing practitioners.
  • Support the Dementia Project by planning and coordinating cognitive stimulation therapy sessions and peer support for those living with dementia.
