Job summary

City of York Council is seeking caring, proactive individuals to join our community-based team to help adults live independently and stay connected in their own homes. You will work with people, families and partners to build confidence, resilience and practical life skills while promoting wellbeing and inclusion.

We offer induction, ongoing development, supervision and a commitment to safeguarding, with a DBS check and CEFR-level English requirements.

Job description

Do you want to help people stay independent, healthy and connected to their communities.

Due to the expansion of our successful service, we are looking for caring, motivated and proactive individuals to join our growing community-based team.

This exciting role focuses on supporting adults with mental health, physical health conditions and long-term health needs to live independently in their own homes and remain active within their communities.

Our approach is centred on prevention, early intervention and promoting wellbeing, helping people maintain their independence and reduce the need for more intensive support services.

As a Support Worker, you will work closely with individuals, their families and partner organisations to provide person-centred support that builds confidence, resilience and practical life skills.

You will empower people to make informed choices, achieve personal goals and retain control over their daily lives.

The role encourages individuals to access local services, build social connections and take part in activities that promote health, wellbeing and community involvement.

You will contribute to the development and review of person-centred support plans and risk assessments, maintain accurate records, monitor outcomes and ensure individuals are safeguarded at all times. The role requires sound judgement, the ability to work independently and a commitment to delivering high-quality support that focuses on strengths, aspirations and positive outcomes.

As this role is public facing, applicants will need to demonstrate, at interview, their competency to converse and provide advice and guidance to members of the public, in spoken English to CEFR level C2: Can express themselves spontaneously at length with a natural conversational flow, avoiding or backtracking around any difficulty so smoothly that the person with whom they are conversing is hardly aware of it.
