Team Manager – Hospital at Home Midlothian
Midlothian Health and Social Care Partnership
Base: Midlothian Community Hospital
An exciting opportunity to lead Hospital at Home in Midlothian

An exciting opportunity has arisen for an experienced and motivated registered nurse to join Hospital at Home Midlothian as Team Manager.

Hospital at Home provides multidisciplinary assessment, treatment, monitoring and clinical decision-making for adults who would otherwise require admission to an acute hospital, or who can be supported to leave hospital sooner with enhanced care in their usual place of residence.

This is a key operational and professional leadership role within a developing service that supports admission avoidance, timely discharge and improved patient flow across the wider health and care system.

The role

As Team Manager, you will provide visible, compassionate and effective leadership to the Hospital at Home nursing team. You will be responsible for the day-to-day management, coordination and governance of the service, ensuring that patients receive safe, effective and person-centred care.

You will lead and manage a skilled workforce comprising Advanced Nurse Practitioners, Band 6 and Band 5 Registered Nurses, and Band 4 support staff. Your responsibilities will include workforce deployment, workload allocation, recruitment, attendance management, staff development, performance management and maintaining an appropriate skill mix within available resources.

Working closely with the Clinical Nurse Manager, Hospital at Home medical team, Chief Nurse and wider multidisciplinary partners, you will support the continued development of the service and help translate strategic priorities into safe and sustainable operational delivery.

The post holder will also contribute to:

  • Clinical and staff governance
  • Workforce planning and service redesign
  • Quality improvement, audit and assurance
  • Risk identification and management
  • Development and implementation of local procedures and pathways
  • Management of delegated staffing and non-pay resources
  • Investigation of complaints and implementation of learning
  • Staff supervision, development and succession planning
  • Delivery of safe, effective and financially sustainable services
  • Partnership working across acute, primary, community and social care services

The post holder will deputise for the Clinical Nurse Manager when required.

About you

We are looking for an experienced registered nurse who can demonstrate strong clinical leadership, sound professional judgement and the ability to lead teams through change.

You will be an approachable and credible leader who can motivate staff, build positive relationships and maintain a clear focus on quality, safety and patient experience. You must be able to work confidently in a complex and fast-paced environment, balancing changing operational demands with workforce, governance and financial responsibilities.

You should have:

  • Current registration as a nurse with the Nursing and Midwifery Council
  • Relevant clinical experience and specialist knowledge applicable to community-based acute care
  • Evidence of further education, including an appropriate postgraduate or community nursing qualification
  • Relevant leadership and management experience
  • Evidence of continuing professional development
  • Experience of managing staff, resources, performance and service delivery
  • Well-developed communication and interpersonal skills
  • The ability to influence, negotiate and work collaboratively across professional and organisational boundaries
  • Experience of clinical governance, quality improvement, audit and risk management
  • Effective digital and information-management skills

Leadership or management development, such as Leading Better Care or an Institute of Leadership and Management qualification, would be advantageous.

Why join us?

This role offers an excellent opportunity to:

  • Lead an established multidisciplinary service supporting people to receive acute care closer to home
  • Shape the continued development of Hospital at Home across Midlothian
  • Work collaboratively with experienced nursing, medical, allied health professional, pharmacy, primary care and social care colleagues
  • Contribute directly to admission avoidance, early supported discharge and improved system flow
  • Develop your leadership experience within NHS Lothian and Midlothian Health and Social Care Partnership
  • Make a measurable difference to patients, families and local communities

You will be supported by the Clinical Nurse Manager and will work within an experienced community nursing leadership structure. You will also have important professional and clinical working relationships with the Hospital at Home medical team and the Chief Nurse.
