Job summary

Band 5 Community Staff Nurse - 0-19 Service

Are you passionate about making a difference in the lives of children, young people, and families? We're looking for a dedicated and compassionate Band 5 Community Staff Nurse to join our dynamic 0-19 team.

As part of the wider community nursing and primary healthcare team, you'll work under the guidance of the Specialist Community Public Health Nurse (Team Lead), delivering a range of services including health surveillance, health promotion, immunisations, and providing support and advice to families with children and young people aged 0-19.

You'll collaborate closely with statutory and voluntary partner agencies to achieve the best health outcomes for children and families. Using evidence-based practice, you'll contribute to delivering a safe, effective, and high-quality service aligned with professional standards.

This role involves supporting the implementation of the 0-19 Healthy Child Programme within a progressive universal model, tailoring interventions based on assessed needs and ensuring seamless partnership working across agencies.

Main duties of the job
  1. Support the delivery of the 0-19 Healthy Child Programme, providing high-quality, innovative services that help children, young people, and families thrive, working independently to support clients.
  2. Manage your designated workload within your specialty with support, advice, and supervision from a SCPHN clinician, assessing, planning, delivering, and evaluating patient-centered care within your competence, adhering to professional protocols and evidence-based practice.
  3. Assess, plan, implement, and evaluate individual and family-focused care in partnership with other agencies under the delegation of a SCPHN.
  4. Establish and maintain partnerships with families and agencies in a multi-agency, integrated environment.
  5. Participate in locality meetings.
  6. Maintain a professional service image and act as a role model for staff.
