Rochester Regional Health is seeking a Licensed Practical Nurse (LPN) to provide professional patient care in the Long Term Care unit at DeMay Living Center in Newark, NY. You will supervise CNAs, document care, and help coordinate treatment plans.

This full-time evening/night shift includes every other weekend, with competitive pay, shift differentials, and a path for advancement from LPN to RN.

Qualifications

• Graduate from an accredited School of Practical Nursing.
• Current NY State license as Practical Nurse.
• BLS certification from the American Heart Association.

Overview

Job Title: Licensed Practical Nurse (LPN). Department: Long Term Care. Location: DeMay. Hours Per Week: 40. Schedule: Evenings/Nights, Every other weekend.

Summary

As a Licensed Practical Nurse, you will provide professional patient care to long‑term and post‑acute care residents. You will direct and monitor the care provided by CNAs and consistently ensure proper documentation and recording is taking place. Your ability to lead is crucial in both the success of the CNAs and the care of the patients.

Key Responsibilities

• Responsible for timely, accurate medication administration of assigned team and concurrent required documentation.
• Performs treatments and documents progress of same as per physician orders and facility protocol.
• Assures concise, pertinent and complete resident care documentation using computerized medical record process. Assures the initiation of appropriate care pathway tools.
• Directs CNA assigned to the team in the timely completion of quality individualized personal care in compliance with resident’s individual care plans. Ensures CNA documentation is accurate and complete.
• Responsible for receiving/giving relevant reports, both written and verbal from/to appropriate staff as per departmental protocols.
• Contacts physicians, transcribes orders and follows up with appropriate action to institute orders in timely manner.
• Assures proper monitoring of residents and relevant condition status are reported to charge nurse/supervisor and/or physicians in a timely manner.
• Completes pre‑reviews of medication administration records as per facility protocols.
• Completes required MDS documentation and all related observations in a timely manner.
• Supports CNA team through assisting in direct ADL care of residents.

Minimum Qualifications

Graduate from an accredited School of Practical Nursing. 1 year experience preferred.

Licenses / Certifications

Current licensure in the State of New York. BLS – Basic Life Support (American Heart Association).

Education & Certifications

Education: LPN – Licensed Practical Nurse (New York State Education Department).

Physical Requirements

Medium work: Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; requires frequent walking, standing or squatting.
