Advantmed seeks a licensed Nurse Practitioner to conduct structured in-home wellness and risk-adjustment assessments. This PRN role emphasizes autonomy, accountability, and consistent visit availability with strong operational support.

Responsibilities include in-home annual wellness visits, histories, exams, and EMR documentation, with mileage reimbursement and monthly travel within a defined radius. Flexible weekdays and no evenings or weekends are offered.

Qualifications

• Active, unencumbered NP license in the applicable state.
• 3+ years of patient care experience preferred.
• Comfortable with EMR systems and independent work.

Responsabilités

• Conduct in-home Annual Wellness Visits and chronic condition assessments.
• Perform focused histories, exams, vitals, medication reviews, and screenings.
• Educate members on preventive care and chronic disease management.
