Soleo Health is seeking a Per-Diem Infusion Nurse to support infusion patients in the home or alternate sites. The role requires RN licensure in the state and experience with infusion therapy, with flexible schedules and on-call rotation.

The Infusion Nurse will assess, educate, and coordinate care with the patient, care partners, and the care team, ensuring therapy safety and adherence to protocols in various home environments.

Qualifications

• Experience in infusion therapy and managing home-care patients.
• Comfort with on-call rotations including nights and weekends.
• Proficient in coordinating with care teams and documenting patient care.

Responsabilités

• Provide direct infusion nursing care and education to patients.
• Assess patients and develop care plans for home infusions.
• Coordinate with pharmacists, prescribers, and care partners for safe therapy.
