Trillium Health Resources is recruiting a Community Resource Specialist I to help connect individuals with community resources and coordinate referrals across North Carolina’s South-Central region.
The role supports access to social drivers of health, tracks outcomes, and collaborates with partner organizations. Requires a high school diploma or GED, one year in HHS, and a valid driver’s license; travel within the catchment is necessary.
