Franciscan Health Munster Campus seeks a Clinical Staff Pharmacist to provide pharmacotherapy services across neonatal to geriatric patients. The role includes preparing and supervising routine and sterile preparations, and collaborating with physicians and patients to optimize therapy and costs.

PRN shifts are available in Munster and nearby Michigan City. Day shifts, no weekends. The team aims to deliver high-quality, cost-effective pharmaceutical care with ongoing education and compliance.

Qualifications

• Bachelor's Degree required.
• Doctorate (PharmD) preferred.
• 1 year hospital pharmacy or infusion experience preferred.
• Experience using Epic EHR preferred.
• Registered Pharmacist (RPh) license required.
• BLS certification as required by unit.

Responsabilités

• Consult with and advise healthcare professionals and patients on medication selection, procurement, distribution and safe use.
• Maintain pharmacy records, inventories, controlled substances, and registries.
• Manage disposition of controlled substances in line with regulations.
• Monitor drug therapy for contraindications and interactions.
• Provide clinical pharmacy services including pharmacokinetic consults, renal dosing, nutrition management, anticoagulation, neonatal/pediatric/geriatric dosing, route optimization and antimicrobial stewardship.
• Participate in in-services, trainings, and staff meetings to uphold standards and policies.

The Clinical Staff Pharmacist provides generalized pharmacotherapy services to neonatal, pediatric, adolescent, adult, and/or geriatric patient populations. The position prepares or supervises the preparation of routine pharmaceutical items, and compounded sterile and non-sterile preparations. This position communicates and collaborates with physicians, Clinical Pharmacy Specialists, and patients, to provide clinical consultation, monitor drug therapy, and offer cost-effective alternatives, as appropriate.

PRN - Shifts available in Munster and Michigan City. Day shift, no weekends.

WHAT YOU CAN EXPECT

• Consult with and advise other Healthcare Professionals and patients on matters pertaining to selection, procurement, distribution, and safe and appropriate use of medications.
• Maintain records, such as pharmacy files, inventories, controlled substance records, and registries.
• Manage the disposition of controlled substances and other dangerous drugs in accordance with legal and regulatory requirements.
• Monitor drug therapy regimen for contraindications, drug-drug interactions, drug-food interactions, allergies, appropriateness of drug and dose.
• Provide clinical pharmacy services, as outlined in approved hospital policies, including but not limited to: pharmacokinetic consults, renal dosing, parenteral nutrition management, anticoagulation management, neonatal/pediatric/geriatric dosing management, route optimization, antimicrobial stewardship, and reviewing medications at transitions of care.
• Participate in scheduled in- services, educational presentations, computer-based training, and staff meetings, to ensure ongoing understanding of and adherence to professional standards, regulatory requirements and organizational policies and practices.

QUALIFICATIONS

• Required Bachelor's Degree
• Preferred Doctorate
• 1 year Hospital Pharmacy Experience or Infusion Pharmacy Experience Preferred
• Experience using Epic EHR Preferred
• Registered Pharmacist (RPh) - State Licensing Board
• Basic Life Support Program (BLS) - American Heart Association As required by unit
