Signify Health, part of CVS Health, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations and bridge gaps in care.

You will travel locally to member homes, conduct comprehensive assessments using an iPad-based workflow, and perform diagnostic and preventive services as ordered by health plans. This part-time, per diem role offers flexibility and competitive pay.

Formación

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.

Responsabilidades

• Provide in-home health evaluations to plan members.
• Travel locally and within licensed states as needed.
• Use an iPad and clinical workflow during visits.
• Deliver diagnostic and preventive services per program.
