Jobtailor is seeking a clinician to conduct online Video Visits with health plan members, primarily via telehealth platforms. The role requires full-time availability and a commitment to closing care gaps while maintaining KPI compliance.

The ideal candidate is a board-certified NP or PA with experience in Signify Health In-Home Health Evaluations and telehealth. State licensing must be current, with ability to obtain multi-state credentials as required.

Responsibilities

• Conduct online “Video Visits” with health plan members
• Engage with health plan members primarily online, helping to close gaps in care
• Provide 40 hours of availability weekly
• Execute a strong visit completion rate
• Maintain compliance with operational, clinical, and member experience key performance indicators

Requirements

• Full‑time, board‑certified nurse practitioners or physician assistants
• Experience with Signify Health In‑Home Health Evaluations, Video Visits, telehealth visit delivery
• Maintain training, affiliation, and compliance with Video Visit requirements
• Be trained and credentialed for all health plan clients in primary coverage state(s)
• Maintain active state licenses in coverage areas (FNP, AGNP certification preferred for NPs)
• Able to obtain and maintain multi‑state licenses as required
