FOX Rehabilitation is seeking a licensed Speech Language Pathologist to provide in-home, senior living, and virtual care to older adults in Mayfair, PA and surrounding communities. You will deliver one-on-one therapy, perform evaluations, and develop individualized plans of care, while collaborating with an interdisciplinary team.

The role emphasizes autonomy, flexible scheduling, and meaningful patient outcomes, with opportunities across in-home and community settings and telehealth support.

Qualifikationen

• Valid Speech Language Pathology license in the state(s) of practice.
• Current Certificate of Clinical Competence (ASHA CCC).
• CPR certification is current and CPR-certified.
• Proficiency with electronic medical records (EMR).

• Provide one-on-one Speech Language Pathology to older adults in home or senior living settings.
• Perform evaluations, develop care plans, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, mobility and independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.

Responsibilities

• Provide one-on-oneSpeech Language Pathologyto older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)

• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment

• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence

• Document effectively within FOX’s EMR systemin accordance withregulatory standards

• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists

• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place

• Provide telehealth / virtual care whenappropriate toextend access and flexibility

Qualifications

• ValidSpeech Language Pathologylicense in the state(s) of practice, or eligibility to obtain

• Degree from an accreditedSpeech Language Pathologyprogram

Physical Requirements

FOX clinicians must be able to meet the physical demands of in-home and community-based care, including:

• Exerting medium physical effort, including lifting 25–50lbsoccasionally, 10–25lbsfrequently, and up to 10lbsconstantly

• Performing CPR

• Providing moderate to maximumassistancewith patient transfers and bed mobility

• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities

• Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model

• Wearing PPE whenrequired
