FOX Rehabilitation is seeking licensed Speech-Language Pathologists to deliver in-home and senior living care in Fishers and Noblesville, IN. You will conduct evaluations, develop individualized plans, and provide evidence-based therapy to older adults in their homes or communities.

You'll have autonomy over your schedule, opportunity to use telehealth, and collaborate with an interdisciplinary team to improve patients' quality of life and independence.

Qualifikationen

• Valid Speech Language Pathology license in the practicing state(s) or eligibility to obtain
• Degree from an accredited Speech Language Pathology program
• Current CPR certification
• Current Certificate of Clinical Competence (ASHA CCC)

• Provide one-on-one speech language pathology services to older adults in private residences or senior living communities
• Perform evaluations, develop individualized care plans, and deliver evidence-based treatment
• Deliver therapy sessions to promote wellness, mobility, and functional independence
• Document effectively in FOX’s EMR system per regulatory standards
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists)
• Educate patients, caregivers, and staff to promote safety and aging in place
• Offer telehealth/virtual care when appropriate to extend access

Responsibilities

• Provide one-on-oneSpeech Language Pathologyto older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)

• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment

• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence

• Document effectively within FOX’s EMR systemin accordance withregulatory standards

Qualifications

• ValidSpeech Language Pathologylicense in the state(s) of practice, or eligibility to obtain

Physical Requirements

FOX clinicians must be able to meet the physical demands of in-home and community-based care, including:

• Exerting medium physical effort, including lifting 25–50lbsoccasionally, 10–25lbsfrequently, and up to 10lbsconstantly

• Performing CPR

• Providing moderate to maximumassistancewith patient transfers and bed mobility

• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities

• Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model

• Wearing PPE whenrequired
