Zusammenfassung

CoAct are seeking Specialist Support Coordinator to join us in this exciting opportunity, to deliver high-quality, person-centred NDIS Support Coordination services to participants with complex and multiple support needs. This role will support participants to build capacity, exercise their choice and control, ensuring they are heard, whilst navigating the NDIS and broader service systems.

CoAct are seeking Specialist Support Coordinator to join us in this exciting opportunity, to deliver high-quality, person-centred NDIS Support Coordination services to participants with complex and multiple support needs. This role will support participants to build capacity, exercise their choice and control, ensuring they are heard, whilst navigating the NDIS and broader service systems.

Exploring the Role

The Specialist Support Coordinator manages complex caseloads, coordinates multi-agency supports, and contributes to high-quality service quality and high standards of compliance.

Your key responsibilities include, but are not limited to:

  • Support participants to understand and implement their NDIS plans by promoting participant choice, control and decision-making.
  • Ensure supports are coordinated in a way that maximises positive participant outcomes, value for money, and plan effectiveness.
  • Ensuring complex and high-risk cases are managed proactively, with early identification and mitigation of risks.
  • Develop and maintain effective relationships with service providers and stakeholders.
  • Maintain timely, accurate and compliant case notes and documentation.
  • Promote accountability and high standards by contributing to team planning initiatives and mentoring colleagues to achieve quality outcomes.
Skills and Experience
  • Passionate and values-driven to provide high quality support, striving for positive outcomes for participants.
  • Strong time and workload management, ensuring priority tasks are completed in a timely manner and within service standards.
  • Ability to build strong, professional relationships with key stakeholders.
  • Strong understanding of the NDIS and service systems.
  • Experience working with complex or high-risk participants.
  • Hold a relevant tertiary qualification (e.g. Social Work, Community Services or related discipline) or equivalent experience within disability, community or social services.
