FOX Rehabilitation is seeking a licensed Physical Therapist to provide one-on-one geriatric care in patients' homes and senior living communities in Chicago, IL. The clinician will develop customized plans, perform evaluations, and deliver evidence-based treatment within the Geriatric House Calls model, with autonomy over schedule and the option for telehealth.

Qualifikationen

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

• Provide one‑on‑one physical therapy to older adults wherever they call home (private residences, senior living communities, independent living, assisted living, memory care)
• Perform evaluations, develop individualized plans of care, and deliver evidence‑based treatment

• Document effectively within FOX’s EMR system in accordance with regulatory standards

Physical Requirements

FOX clinicians must be able to meet the physical demands ofin‑homeandcommunity‑basedcare, including:

• Exerting medium physical effort, including lifting 25–50lbsoccasionally, 10–25lbsfrequently, and up to 10lbsconstantly

• Performing CPR

• Providing moderate to maximumassistancewith patient transfers and bed mobility

• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities

• Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model

• Wearing PPE whenrequired

In Illinois, the standard base pay range for a Full‑Time role is $65,000 - $105,000 annually with an opportunity to earn more. Part‑Time and FOX Flex (PRN) will be pro‑rated.
