MTM Pharmacist – Help Patients Get the Most Out of Their Medications

Responsibilities
  • Conduct comprehensive medication reviews (CMRs) with patients to identify medication-related problems.

  • Document interventions clearly and maintain compliance with MTM program requirements.
  • Educate patients on safe, effective use of their medications and address barriers to adherence.
Qualifications
  • Education: Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy degree.
  • Licensure: Active and unrestricted pharmacist license in the U.S.
  • Experience: Retail, hospital, ambulatory care, or prior MTM experience is a plus—but all licensed pharmacists are encouraged to apply, including new grads.
  • Skills: Strong communication, patient counseling, and ability to work independently.
