Signature HealthCARE Bremen is seeking a Medication Aide to accurately prepare, administer, and document medications for residents in a 97-bed setting. You will follow state guidelines, maintain stock, and support daily living activities under licensed supervision.

Required certifications include Medication Aide and Nursing Assistant. Strong attention to detail, clear communication, and teamwork are essential. EHR familiarity is preferred; agency procedures and patient safety come first.

Qualifikationen

• Current Medication Aide Certification and Nursing Assistant Certification in the state of practice.
• Strong attention to detail with accurate medication preparation and documentation.
• Knowledge of facility policies and state regulations governing medication administration.
• Effective verbal and written communication skills.
• Ability to work collaboratively within a nursing team and under licensed staff.
• Customer service oriented with ability to work under pressure.
• Approachable, flexible and adaptable to change.
• Familiarity with EHR systems preferred.

How You Will Make a Difference

• Accurately and safely prepare, administer, and document oral and/or topical medications as ordered by the attending physician, per state and federal guidelines.
• Adhere to the 5 rights of medication administration (right resident, right mediation, right does, right route, right time)
• Observe and document medication administration, including any refusals, omissions, or errors, and promptly notify the Charge Nurse.
• Monitor medication supply levels and coordinate with the Charge Nurse to ensure adequate stock is maintained.
• Adhere to controlled substances, narcotic recordkeeping, and medication destruction requirements per state and federal guidelines.
• Assist with activities of daily living (ADLs) and provide CNA-level care as needed and directed.
• Participate in quality assurance and process improvement activities as requested.
• Perform other duties as assigned by the Charge Nurse or Director of Nursing.
